We have an exciting opportunity to join our team as a Social Worker.
In this role, the successful candidate Under the supervision of the Associate Program Director, and the Program Psychiatrist, the Social Worker works 2 days per week with patients in the Wards Island and Barbara Klienman emergency shelter /residences providing mental health and substance use services in the homeless setting. The Social Worker works 3 days per week as a provider in the Primary Care Integration Health Center Network, at either the Sunset Terrace, Park Ridge or Caribbean American family health centers. These services are an Expansion of the present HRSA funded Opioid SA services. The Social Worker is a 1.0 FTE employee, totally compensated through AIMS funding. The social worker has the main function of providing screenings, assessments, and a brief model of psychotherapy to patients who screen positive for mental health disorder(s) and/or substance use disorders (Opioids and other drugs) works closely with clinical staff as a member of the patient provider team, and provide feedback to the referring PCP develop treatment plans and refers to intensive services in the community, when necessary.
- Participates in the site team meetings, including conducting daily huddles, conducting MH staff development in service trainings and other features of SA screenings of all patients at the sites.
- Conducts previsit screenings and determine access to care
- Provides clinical feedback and coordinate care with the referring PCP
- Documents psychotherapy visits in the EHR
- Develops a treatment plan in the EHR based on the patients behavioral health issues and/or substance use disorders
- Provides brief psychotherapy using evidence based treatment (Problem Solving Therapy)
- Conducts psychosocial assessment and other screenings
- Receives clinical patients from the PCP when primary care patient screens positive for mental health or substance abuse conditions (warm handoff)
To qualify you must have a Masters in Social Work (MSW) degree from an accredited School of Social Work.
1 year previous professional related experience or graduate level mental health experience.
Required Licenses: Lic Master Social Worker
Licensed Clinical Social Worker (LCSW)
Qualified candidates must be able to effectively communicate with all levels of the organization.
