SUNY Upstate Medical University Clinical Care Manager in Syracuse, New York

Assists clinical team in developing individualized care plan for patients that addresses age specific, physical, psychosocial, cultural, and educational needs of patient, including goals, barriers to goals, and strategies to overcoming barriers. Makes timely and appropriate referrals. Assures patient choice by providing list of available resources. Consistently collaborates and communicates with other team members. Conducts pre visit planning, patient outreach, and follow up. Utilizes evidence based interventions, treatments, and appropriate interdisciplinary resources to meet complex patient/family needs. Obtains data that integrates patient's diagnoses, physical/psychosocial findings, patient/family perceptions of illness, learning, needs, and culture. Perform other relevant duties as assigned to include coverage for other care managers and back-up responsibilities. Primary assignment will be at UPAC and Upstate Pediatrics. R.N. NYS license or eligible, Bachelor's degree in Nursing or healthcare related field, plus minimum of three (3) years RN nursing experience required. Pediatric and ambulatory experience preferred. Master's degree in Nursing or healthcare related field and prior acute care case management experience a plus.
